What exactly is pharmaceutical care?

The definition of pharmaceutical care, used by NuPhaC, is based on the PCNE definition: “the care of individuals in order to optimize medicines use and improve health outcomes”. Only we focus on nurses’ contribution instead of the contribution of the pharmacist. We do respect the unique contribution of each professional group involved, and aim for a strong collaboration with the focus improving health outcomes. Togehter with 'Pharmaceutical Care Network Europe' (https://www.pcne.org/) we look at how we can help each other to help the patient.
